Rotary District 5960 Awards Submission Instructions (2025-2026)

Step 1: Access Award Criteria & Forms

• Find the Award Criteria, Checklist, or Signature Project Recognition links in the right sidebar.
• Click each link to open the document in PDF format.
• Choose your preferred submission method:
  • Online Submission → Proceed to Step 2.
  • Email Submission → Skip to Step 3.

Step 2: Complete the Online Submission Form

• Fill out the required fields in the online submission form. (see below for links)
• New for 2024-25: No narrative or photos are required.

Step 3: Finalize the Checklist

• Ensure all required checklist items are completed.
• Review the information with club members for accuracy, if needed.

Step 4: Submit via Email (Optional Alternative to Online Submission)

• If submitting via email, complete each award category’s PDF form(s).
• No narrative or photos are required.
• Send the completed form(s) to:
  Rotary5960@Gmail.com 

Important Note:

You must only submit using one method (online OR email), not both.
